Magna Home Health Care, Inc., is a licensed, Medicare and Medicaid certified Home Health Agency, locally owned, managed and operated by health care management professionals. We also hold a contract with the Department of Human Services Developmental Disability Services Division for Nursing and Rehab services.

SERVICES PROVIDED:
We have a staff mix of Registered Nurses, Licensed Practical Nurses and Certified Home Health Aide. We offer services which include Physical Therapy, Occupational Therapy, Speech Therapy and Medical Social Services, and more.

SUPERVISORY VISITS:
All cases will be supervised by a Registered Nurse. The nurse will make, at a minimum, one supervisory home visit per month on the LPN, and supervisory visits every 14 days on the CHHA. The number of supervisory visits will be based on the amount of service being provided, complexity of the case, and the level of care taken into consideration.

SERVICE COVERAGE:
The Patient must be home bound (leaving home requires considerable, taxing effort)
The Patient must require skilled, intermittent services (a medically predictable, recurring need for service)
The attending Physician must order the services.
All initial referrals will be evaluated by a Registered Nurse.

ON-CALL:
Magna Home Health Care, Inc., will always have a Registered Nurse on-call to handle any medical emergencies, questions regarding services, or staffing issues.
